Like many projects, this project started out to meet a need: we were teaching classes of dental hygiene, dental and post graduate dentists and could not ﬁnd a textbook in statistics designed with the dental health professional in mind. So, we started to write a brief syllabus. We realized that most dentists will not become researchers, however, all will become consumers of research and will need to understand the inferential statistical principles behind the professional journals they read.

The goal of Biostatistics for Oral Healthcare is to give the reader a conceptual understanding of the basic statistical procedures used in the health sciences. Emphasis is given to the rationales, applications, and interpretations of the most commonly used statistical techniques rather than on their mathematical, computational, and theoretical aspects.

Achieving an effective level of communication in a technical book is always a difﬁcult challenge. If written at too low a level, a book will not really explain many important points and risks insulting intelligent readers as well as boring them. However, if written at too advanced a level, then a book may have difﬁculty ﬁnding an audience.

We have tried to write at a fairly elementary level, but have not hesitated to discuss certain advanced ideas. And we have gone rather deeply into a number of important concepts and methods.

DESCRIPTIVE STATISTICS

The content of Chapters 1 through 5 includes the basic concepts of statistics and covers descriptive statistics. Included are discussions of the rationale for learning and using statistics, mathematical concepts and guidelines for studying statistical concepts (Chapter 1); organizing and graphing data (Chapter 2); describing distributions, measures of central tendency, and measures of variation (Chapter 3); random variables including both discrete and continuous (Chapter 4); and the three most useful distributions in the health sciences: binomial distribution, Poisson distribution and normal distribution.

INFERENTIAL STATISTICS

The discussion of inferential statistics begins in Chapter 6 where the recurring question of sufﬁcient sample size is addressed. Chapters 7 through 9 covers how to determine appropriate sample size for a population and compute conﬁdence intervals as well as hypothesis testing and estimation for one-sample and two-sample cases for the mean and other statistics. Chapter 10 describes hypothesis testing for categorical data.

ADVANCED TOPICS

We began the text with a review of basic mathematical and statistical concepts and we end the text with some of the more sophisticated statistical concepts and procedures. We include discussions of one-way and two-way analysis of variance as well as a description of parametric statistical methods used for data analysis. And ﬁnally, we discuss non-parametric statistics and survival analysis that are particularly useful in dental and medical clinical trials.

It is our sincere hope that the conceptual approach of this book will prove to be a valuable guide for dental health professionals in basic introductory courses as well as more advanced graduate level courses. We hope that we have been successful in providing an integrated overview of the most useful analytical techniques that students and practitioners are bound to encounter in their future studies, research activities and most importantly, as consumers of evidence based dentistry.

Today's miracle medicine scenario of genetic engineering and regenerative medicine is a timely reminder of dentistry's relatively small yet indispensable, role in advancing health care. The dental profession has been in the body 'spare parts' business for a very long time, but without the anguish inherent in the tricky ethical questions associated with organ transplantation. The scope of dental practice has evolved enormously since the era of the barber surgeon. Oral surgery remains, however, not only a traditional skill in dentistry but also a core skill for all dental surgeons regardless of their area of specialism, and therefore it is an important part of the undergraduate curriculum and general professional training.

Over the years, as the medical status of the population has become more complex and surgical expertise has increased, oral surgery has evolved into identified subspecialties. These include maxillofacial surgery, which, in the UK, is a specialty of medicine; oral surgery, which embraces maxillofacial trauma and orthognathic surgery; and dentoalveolar surgery, which is designated surgical dentistry by the General Dental Council in the UK. The first two - maxillofacial surgery and oral surgery - are the remit of specialists, whereas all dentists are expected to be competent in dentoalveolar surgery. A sound knowledge of basic surgical principles is a prerequisite to the practice of any of these areas of surgery.

This text includes a consideration of general surgical principles, specialist surgical areas and minor oral surgery. The section on general surgical principles has been written mainly by general surgeons and provides core knowledge that informs the safe practice of surgery. It will be of practical help to those working as senior house officers in maxillofacial surgery wards. This section also considers cross-infection control and provides an overview of both general anaesthesia and conscious sedation.

The second section includes chapters on individual areas of specialist surgical practice of interest to oral and maxillofacial operators, written by experts. These are written to provide insight into these relevant areas of surgical practice so that the dentist can be confident in the information he or she provides to patients and can also make appropriate referrals. This section is not intended to inform practice in these areas and so it is short and readable.

The third section - oral surgery - is a practical guide to the practice of dentoalveolar surgery or surgical dentistry. It provides core information required to complete the undergraduate curriculum.

The integrated nature of this text, which includes general and oral surgery, is a companion to the Textbook of General and Oral Medicine, and is recommended for students studying human disease earlier in the undergraduate curriculum and, subsequently, oral surgery in the clinical years. Although intended primarily for undergraduate students, the book also provides a comprehensive range of information for those preparing for membership examinations and will be a useful bench book in a dental practice environment.

The authors have taken great pleasure and satisfaction in compiling this text, which is unique in bringing together succinct knowledge on the whole scope of surgical practice in dentistry. It is hoped that the reader will also be pleased and satisfied.

The third edition of Clinical Aspects of Dental Materials: Theory, Practice, and Cases continues to provide a dental materials background that emphasizes the clinical aspects of dental materials. Hopefully, the student will become more familiar with the practice of dentistry through the use of this textbook.

Organization

The text is divided into three distinct parts: Theoretical Perspectives, Laboratory and Clinical Applications, and Case Studies. Chapters have been updated throughout the book and a few have been reorganized for easier understanding. Five chapters reflect significant revisions, including Chapter 4, Adhesive Materials; Chapter 5, Direct Polymeric Restorative Materials; Chapter 16, Polishing Materials and Abrasion; Chapter 17, Tooth Whitening; and Chapter 31, Clinical and Laboratory Aspects of a Whitening Tray.

Two new chapters have also been added to Part II. Chapter 36, “Finishing and Polishing Composite Restorations,” provides a step-by-step technique and suggests a variety of instruments that will achieve the desired result. It has been presented in this format because there are many finishing and polishing instruments and abrasives, and each dentist has a preference in performing this procedure. Chapter 37, “Tips for the New Hygienist,” is written by a contributing author who wanted to share some valuable information for the future dental hygiene practitioner. The text authors wholeheartedly agreed, so this short chapter is added at the end of this section. Part III, which highlights clinical case studies, has had two new cases added, for a total of six.

Some sections of many chapters are labeled as optional. The instructor should feel free to designate other sections as optional as well. The outline format is designed to provide a clear organization of the topic and to facilitate the reading of both the theory and laboratory chapters.

Features

Part I, which includes Chapters 1 through 22, now has more Review Questions. Part II chapters—those addressing laboratory and clinical applications, and specifically 23 through 36—have had Review Questions added. These are designed for not only course review, but for review for national boards as well.
“Tips for the Clinician” and “Precaution” boxes are included in almost all of the chapters in Part II. Summaries of procedures are also highlighted in boxes to provide easy review.
This new edition of Clinical Aspects of Dental Materials has a 4-page color insert that contains several photographs with descriptive captions. Being able to see color in certain dental materials, matching restorations, and differences after tooth whitening will provide for better understanding.
Answers and Justifications for all chapter and clinical case review questions are provided in Appendix 1. At the end of the chapters in Part II, the text refers the reader to Appendix 2, where the Skill Performance Evaluation Sheets are located. They are perforated for easy removal and can be folded for separate evaluations by the student and instructor. The instructor can use this section as a dental materials and/or expanded functions lab manual for the course being taught.

Epidemiology draws important understanding from other disciplines to study the distribution and determinants of health-related states and their outcomes in populations. The oral epidemiologists’ charge is to adapt epidemiological techniques for answering questions related to health states in the oral and craniofacial region of the body. It may be argued that because, by definition, epidemiology is a study of health states in populations, it has little role in basic sciences, which are mostly concerned with narrowly focused laboratory-based projects assessing causal relationships. However, epidemiological concepts have major applications in basic science, general clinical research, and public health as well. The phenomenal growth of molecular applications in everyday health care, genome- and stem cell-based reorientation of medical applications and developments in neurobiology to name a few events, have changed the paradigm of epidemiology from being viewed as being remote to basic science. In current day practice, scientific studies, whether those are laboratory-aided computational biology studies, clinical trials, population-based observational studies, hospital-based clinical studies, or computer-based in-silico simulation studies, all use epidemiological principles.
The range of disciplines that oral epidemiology covers is vast, which makes it a truly inter-/multi-/trans-disciplinary subject. Although several excellent books exist that discuss epidemiologic methods and analysis, none deal with oral health issues to serve as a ready-reckoner and quick general reference for oral health students, practitioners, and researchers to help assimilate and organize scientific information for drawing logical inferences. Osteomyelitis of the jaws is still a very unique disease of the facial skeleton that represents a great challenge for the physician as well as the patient being treated, despite all recent advances in diagnosis and evolved treatment modalities. In the past decades the clinical appearance of osteomyelitis cases has changed dramatically. Not only has the average number of cases seen in a maxillofacial unit decreased, but also the clinical picture of the disease itself has changed significantly. Osteomyelitis of the jaws used to be an infectious disease with an often complicated course, involving multiple surgical interventions and sometimes leading to facial disfigurement as a result of loss of affected bone and teeth and the accompanying scarring. The outcome was usually all but certain; hence, prolonged treatment and frequent relapses have been associated with this disease in the past.
Osteomyelitis of the jaws is a disease that has affected mankind since prehistory. In fact, the famous 1.6 million-year-old fossil find of “Turkana Boy” documents this very well. As a 12-year-old prehuman hominid (homo erectus) his nearly complete skeleton clearly showed an osteomyelitis arising from an odontogenic infection around one of his first molar teeth. Paleontologists even conjecture that it was the most likely cause of his premature death.

Today, medical and dental specialists continue to treat osteomyelitis of various types with the recognition that osteomyelitis of the jaws differs significantly from osteomyelitis of the long bones and at other skeletal sites. These differences are due to a different group of pathogens, the presence of teeth, a different blood vessel density, an oral environment, a thin mucosa as opposed to skin, one jaw that is mobile and the other that is fixed, the more frequent presence of foreign bodies, and the commonality of head and neck radiotherapy. These differences are also reflected in the confusing array of terms used to describe the different forms of jaw osteomyelitis, e.g., chronic diffuse sclerosing osteomyelitis, Garré’s osteo-myelitis with Proliferative periostitis, periostitis ossificans, etc., as well as many entities that are not primarily infectious etiologies but develop a secondary osteomyelitis by virtue of exposed bone in the oral cavity such as osteoradionecrosis, osteopetrosis, and bisphosphonateinduced osteonecrosis.
In this book, M. Baltensperger and G. Eyrich begin with a straightforward classification and terminology system that is consistent with the scientific evidence of the different clinical forms of jaw osteomyelitis. They further coordinate the initiating factors, host local and systemic factors, and pathogenesis to the clinical presentation. In a logical manner, useful to the student, resident, and senior clinicians alike, a differential diagnostic methodology is offered, inclusive of clear descriptions of what the present imaging technology can and cannot offer. This is followed by separate chapters on the different microscopic pathologies seen with each type of osteomyelitis and the microbiology of the known pathogens. The next three chapters are focused on treatment. They logically begin with the principles of treatments including the selection of the most appropriate surgical procedure and its follow-up by the selection of the appropriate antibiotic, as well as its correct dosing and the role of adjunctive hyperbaric oxygen in selected cases. The next chapter openly discussed the uniqueness of osteomyelitis involving the temporomandibular joint and its treatment, a disease not often discussed in other texts. The final chapter underscores the practical value of this book. In an innovative manner the authors present 20 case reports of actual cases and how they were diagnosed and further managed. These cases encompass nearly the full spectrum of osteomyelitis of the jaws and support the application of the principles and techniques reviewed in the preceding chapter.
The reader should appreciate the flow of this book, from the history of osteomyelitis to its classification, diagnosis, pathology, imaging, and treatment. The reader should also appreciate that a complex disease, such as osteomyelitis of the jaws with numerous variations, can be simplified in approach while still retaining the comprehensiveness of its diagnosis and treatment. The student can use this book as a comprehensive textbook or on a chapter-by-chapter basis. The experienced clinician can use the book for general review or as a case review as he or she may be confronted with their own challenging case. Both groups will find the case reports to be the realistic element that brings everything together.

Salivary gland neoplasms display a striking range of morphologic diversity that can confound cytologic as well as histologic interpretation. The overlapping cytologic features, the lack of markers to accurately distinguish various tumors, and the overemphasized FNA-induced histologic changes have fostered uncertainty in the role of FNA in the management of salivary gland lesions.

Classifying salivary gland neoplasms may be difficult, as the majority of the tumors arise from epithelial and myoepithelial cells that also have the ability to undergo a variety of metaplastic changes (squamous, mucinous, sebaceous, oncocytic, and chondroid). When a specific diagnosis cannot be made with a high level of certainty, the lesion is best reported by generating a differential diagnosis. Some experts advocate classifying salivary gland lesions into diagnostic categories based on the cell type and extracellular material.
This book, written by two experienced authors, will fulfill the need of those who wish to familiarize themselves with the FNA of salivary glands.
